Position Summary: 

As a Hospice Admissions Nurse you are often the first point of contact in one's journey into hospice services. With deep compassion and empathy, this role educates patients and their families about the benefits of hospice and what to expect along the way. The goal is to assess individual needs to devise a holistic plan of care that incorporates factors such as psychological, spiritual, cultural and medical needs in order to maximize comfort.

  • Reviews medical documentation and provides a complete physical assessment and history of current and previous illness(es)
  • Conducts the initial assessment and devises an optimal interdisciplinary plan of care that incorporates palliative nursing actions
  • Includes the patient and the family in the planning process
  • Participates in revising the plan of care as necessary
  • Provides professional nursing care by utilizing all elements of nursing process
  • Establishes and maintains positive relationships among patients, families, physicians, and other disciplines through timely and effective communications
  • Adheres to policies, industry best practices, and regulations in order to provide a positive patient experience
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.

Requirements

  • Current and unrestricted CA license
  • 1 years of Hospice Experience required
  • Current AHA CPR certification
  • Driver's License
  • Able to travel to our Oakland office for weekly IDG meetings
  • Self-directed and able to work with minimal supervision
